CARE Education

We use a multidisciplinary team approach to provide the best care to treat the whole patient. CARE Clinic patients receive holistic treatment that involves complete medical and lifestyle assessments, laboratory testing as needed, and nutritional counseling. Patients with medical complications may also recieve treatment from other Phoenix Children’s Hospital pediatric specialists in conjunction with clinic practitioners.

Long-term follow-up care may be provided through Phoenix Children’s Hospital and/or a patient’s primary health care provider, depending on the patient’s medical condition and final treatment plan.

Referral criteria for the CARE Clinic

In order to best utilize the resources available through the CARE Clinic, we recommend only referring patients that meet following criteria:

Children and adolescents < 18 years of age
BMI > 97th percentile with or without medical risk factors
BMI 95 -97th percentile with risk factors such as:

HbA1c > 5.7
Fasting glucose > 100
Non-fasting glucose > 140
Elevated liver function test
LDL > 110
Triglycerides > 150

BMI < 95th percentile with the following risk factors:

LDL > 160
TG > 300

If patients do not meet the CARE Clinic referral criteria, patients should be referred to Phoenix Children’s Hospital Endocrinology to be seen by a pediatric endocrinologist in our general endocrinology clinic.
