Preparing For Your First Visit to Sleep Clinic
If a sleep disorder is suspected, your pediatrician may refer you to Phoenix Children’s Hospital’s Comprehensive Sleep Medicine Program. Or you may self-refer by calling us directly to make an appointment.
Before your appointment
Once an appointment is scheduled, you will receive a packet of forms in the mail. You may also download and complete the forms below.
Either way, fill out the forms as best you can and bring the completed copies with you to your first appointment.

What to expect at your first appointment
Your first visit will include an extensive office appointment with either and sometimes both of our sleep medicine team specialists. They will do a full evaluation on your child, talk to you about your child’s medical history, assess overall health and order any necessary tests.
Depending on symptoms, the clinic’s comprehensive exam process includes an initial consultation and, if needed, an overnight sleep study in which oxygenation, sleep quality, staging of sleep, leg movements and EKG rhythms are closely monitored and evaluated.

Follow-up visits
Depending on the severity and complexity of the condition, children with sleep disorders sometimes may need to see the Sleep Medicine team for follow-up appointments.
However, we are available whenever children with sleep disorders and their families need care, emotional or physical support and guidance on sleep disorder resources.
