Requesting patient records

 

To request that a copy of a patient's medical record be released to you or to an organization (i.e., school, day care provider, employer), please complete the Phoenix Children's Medical Records Request Form.  Medical request forms may be returned by fax to (602) 546-1477. They can also be mailed or delivered in person to:

Because a signature is required to release a patient's information, we are not able to process requests received by telephone or e-mail. Photo identification is required when picking up medical records in person. If the entire medical record is requested, the Heath Information Management Department will inform the patient or authorized requestor of the copying charges before processing the request and allowing the requestor to withdraw the request. All information mailed will be subject to postage or other delivery fees.

 

Be sure to complete the following information on the form:

 

  • Patient's name
  • Date of Birth
  • Date(s) the patient was seen at Phoenix Children's Hospital (please specify inpatient or ambulatory clinic if applicable)
  • A request for all pertinent records will include the discharge summary, history & physical, operative/procedure note, consults, and diagnostic test such as lab, x-ray, etc.
  • Include name of person requesting the information, telephone and fax number
  • The date and time of appointment

 

Recieving patient records

 

Records will be mailed in advance of the patient's appointment when possible. Records may be faxed in an emergency situation and/or within one day prior to the patient's appointment.

NOTE: Requests for birth records should be directed to the hospital where the child was born. Phoenix Children's Hospital is a pediatric facility.
