St. Joseph's Transitions Pediatric Services To Phoenix Children's Hospital

 

 

St. Joseph's and PCH strategic partnership

Starting June 1, 2011, CHW/St. Joseph’s Hospital and Medical Center (St. Joseph’s) and Phoenix Children’s Hospital (Phoenix Children’s) are joining their pediatric programs. All patients age 14 and younger who have been treated at St. Joseph’s for medical care and hospital stays will now be seen at Phoenix Children’s. 

 

Note:  Patients age 15 and older may continue to receive care at St. Joseph’s if those services are available.

 

The following St. Joseph’s clinics and services will be moving over to Phoenix Children’s on June 1st:

 

Congenital Heart Center  Neurology
Genetics  Gastroenterology
Nephrology  Pulmonology
Rehabilitation Services  

 

 

What This Means for You and Your Family

 

  •  Until May 31, 2011, your child may continue to receive care at St. Joseph’s.

 

  •  However, starting June 1, 2011, your physician most likely will have moved from St. Joseph’s to Phoenix Children’s.  After June 1st, call 602-933-PCMG (7264) or click here to see if your physician now resides at Phoenix Children's.

What You Need to Do 

 

Starting on June 1, 2011, your child’s care and a copy of your child’s medical records will be transferred to Phoenix Children’s.  If you do not want a copy of your child’s medical records sent to Phoenix Children’s, please call your physician's office. St. Joesph's will keep a copy of your child’s original medical records there at their location.

       
    • If you want your child to see the same physician at Phoenix Children’s and you have questions regarding existing or future appointments, please call 602-933-PCMG (7264).

     

    • If you want your child to see a new physician after your child’s current physician moves to Phoenix Children’s, you will need to choose another physician. When you decide who that will be, you need to give us written permission to send a copy of your child’s medical records to that new physician.  Please call us at [insert phone number] to arrange a medical records release. 

     

    If you decide to choose a new physician, you have these two choices:

     

    • If your child is age 15 or older and St. Joseph’s still has the services you need, you may stay at St. Joseph’s with a new physician.  Please call the St. Joseph’s ResourceLink at 877-602-4111 for more information.

     

          or

     

    • You may choose a new physician outside of St. Joseph’s and Phoenix Children’s network.  Please contact your health insurance plan to get a listing of physicians in the area who will accept your health insurance plan.  This provider directory may be on the plan’s website.

     

    We are working with St. Joseph's to make sure your health plan knows your child’s physician is changing his/her practice location.  And Phoenix Children’s, like St. Joseph’s, is in the provider network for most health plans in the area.
