How to Refer a Patient

The staff at Phoenix Children's Hospital appreciates you entrusting your patient's care to us. We strive to assure you and your patients that they will receive the best pediatric-specific care possible.

There are two ways to refer your patient to us:

• Name
• Age
• Gender
• Diagnosis
• Past medical history
• Previous hospitalizations
• Exposures/immune status
• Current vital signs, respiratory status
• Isolation needed

For assistance with stabilization and transport of critically ill children, this toll free number is available at Phoenix Children's Hospital 24 hours a day, 7 days a week.

Newborn Intensive Care Unit

For assistance with critically ill newborns, contact our NICU to speak with a neonatologist.
