How to Refer a Patient
The staff at Phoenix Children's Hospital appreciates you entrusting your patient's care to us. We strive to assure you and your patients that they will receive the best pediatric-specific care possible.
There are two ways to refer your patient to us:

- Name
- Age
- Gender
- Diagnosis
- Past medical history
- Previous hospitalizations
- Exposures/immune status
- Current vital signs, respiratory status
- Isolation needed
